Geriatric Care Management

Our Care Management team will focus on improving care to help promote quality of life and achieving optimal level of function. 
  • Evaluating, arranging for, and monitoring in-home personnel and care needs. Including home health caregivers, in-home physicians, and medication renewals. 
  • Coordinating medical appointments and arranging for transportation (Nurse will accompany patient if requested.)
  • Identifying social services and programs that could help the care recipient.
  • Making referrals to financial, legal, or medical professionals and suggesting ways to avert problems. 
  • Creating short - and long-term care plans that could include other living arrangements and placement. 
  • Acting as a liaison to families who may be long-distance. 
  • Answering questions and addressing concerns of caregivers and their loved ones. 
  • Arranging for relief or respite care for stressed-out caregivers. 
  • Transition Care from state to state, hospital to home, home to assist living wherever you may transition we help make the move easier. We work with moving companies, cleaning services, real-estate professionals etc...
