Medicaid Waivers

Home and community-based services (HCBS) provide opportunities for seniors and disabled persons, to receive services in their own home or community rather than institutions or other isolated settings. These programs serve a variety of targeted populations groups, such as people with intellectual or developmental disabilities, physical disabilities, and/or mental illnesses. These are state paid programs.


• Examples of care that may be provided by waivers is attendant care, supervision, adult day care, respite care, and homemaker services.


Elderberry can provide services for participates ages 18 and older that are approved though the Medicaid Wavier programs. We accept:

• Humana
• Sunshine
• Florida Community Care

 

How to Get on the Florida Medicaid Waiver Wait List


Step 1:
Call your local area agency on aging (AAA). These agencies are also referred
to as "aging and disability resource centers" (ADRC). In Palm Beach County,
Area Agency on Aging of Palm Beach/Treasure Coast, Inc.

The agency is called “Area on Aging of Palm Beach/Treasure Coast”. Every
county in Florida is assigned to an Area Agency on Aging


The AAA will take down some preliminary information over the phone and will

let you know an approximate time for a follow up phone assessment.


Step 2
:
After the phone assessment you wait and follow the Medicaid guidelines.


Step 3
:
Getting off the list and choosing a company. Each Medicaid Wavier
would prefer you choose their company. However, Elderberry is a local
